Mapping Acknowledgment(Required)

The undersigned has requested information from Clearwater Power Company (“CPC”) regarding location and mapping data of CPC’s equipment, including service and/or transmission lines. The undersigned hereby acknowledges that the information provided is the approximate location of CPC facilities as of the date CPC provides the information and may change in the future with new lines or equipment being added and/or removed from the CPC service area. The undersigned hereby acknowledges that CPC does not provide updated location and mapping data unless requested to do so. Therefore, the undersigned hereby agrees that CPC shall have no liability for the undersigned’s reliance upon and/or use of outdated CPC location and mapping data and that CPC cannot be held liable for any claims or losses of any damages including those of third parties.

Further, I will not allow the requested information to be used in any way for any purpose other than as set forth above. I understand that Clearwater Power Company may impose a charge covering the cost of labor and materials for copies of any documents provided for the information requested. I understand that I may be denied further access to information of the Cooperative and be subject to all other legal remedies available to the Cooperative if I do not comply with my commitments as stated in this request.
